- Approriate documentation was not provided at the time of examination;
- Refused examination room entry due to non-compliant reasons.
PFT Proctored Exam Retake

- Eligibility:
- Exam attempt taken but did not meet the passing exam (71% or higher) requirements.
Non-Proctored Exam Retake
$95
- Eligibility:
- Did not meet the non-proctored exam passing requirements after the third attempt.
- Excludes PFT procotred exam retakes
Exam access will be reset within 24-72 hours of manually validating exam attempt status
Certificate Ship Fee: North America

- Eligibility:
- Lost, damaged, name change request, certificate replacment.
Fee includes per certificate materials, processing and shipping costs.
Certificate Ship Fee: International

- Eligibility:
- Requires physical certificate to be shipped;
